A bar with 25g of total carbs and 18g of sugar alcohols might advertise "only 7g net carbs," which sounds keto-friendly. Whether those 18g of sugar alcohols actually behave the way that label implies depends heavily on which sugar alcohol is used — and maltitol, the most common one, has a glycemic index of around 35, which isn't as low as brands tend to imply.

- Blood sugar impact varies. Erythritol is the best tolerated and has minimal impact. Maltitol — again, the most widely used — behaves more like sugar than brands typically communicate.
- Long-term data is limited. Sugar alcohols haven't been around in processed foods long enough for robust long-term studies.

Step 3: Cross-Reference the Sweetener Claim
Bars that say "no sugar" or "sugar-free" on the front are almost always sweetened with sugar alcohols or artificial sweeteners. There's no such thing as a sweet bar with no sweetener — the question is just which one.
Step 4: Look Up the Specific Sugar Alcohol
Not all sugar alcohols behave the same. Erythritol is generally well tolerated; maltitol is not. A quick search for the specific compound tells you more than the front-of-package claims ever will.
